Overview

The Campus Signed IRS Form W-9 is used for reporting changes to UCSF's business information.

UCSF updates Form W-9 under two conditions:

  1. There are changes to UCSF's business information, which includes changes to federal tax classification, address, and tax identification number (TIN). We do not update UCSF's business information annually.
  2. The Internal Revenue Service (IRS) publishes a new revision to the form. You can verify the current version of the form on the IRS website. See the “Current revision” section at https://www.irs.gov/forms-pubs/about-form-w-9 (opens in new window).
