The purpose of the Sponsored Project Award Set-up Change Request Form is for the Department to request CGA to make one or more of the following changes:
- Create New Project (Separate projects may be necessary for tracking restricted funds, expenses with separate F&A rates, etc.)
- Change F&A Base
- Change F&A Rate
- Change Project Location
- Change Award Dept ID
- Change Project Dept ID
- Add or Update Reporting Roles
- Change Award PI
- Change Project PI
- Please complete all applicable fields, including Exception Dating if necessary and Requestor Information
- Multiple change requests may be combined on one form.
