The purpose of the Sponsored Project Award Set-up Change Request Form is for the Department to request CGA to make one or more of the following changes:

  • Create New Project (Separate projects may be necessary for tracking restricted funds, expenses with separate F&A rates, etc.)
  • Change F&A Base
  • Change F&A Rate
  • Change Project Location
  • Change Award Dept ID
  • Change Project Dept ID
  • Add or Update Reporting Roles
  • Change Award PI
  • Change Project PI
  • Please complete all applicable fields, including Exception Dating if necessary and Requestor Information
  • Multiple change requests may be combined on one form.
