Multi-Location Appointment / Interlocation One-Time Payment Form

A temporary inter-location appointment form is required when an employee at one campus (home campus) is eligible to receive a payment originating from another campus (host campus) for a short or long term duration. The location at which the employee holds their primary employment is the home location. The location that employs the employee on an additional basis is the host location. The host department should prepare a Multi-Location Appointment/Interlocation One-Time Payment Form (formerly known as Form UPAY 560-T).
